  Berkshire West CCG Pre-Diabetes CES

A reminder that this CES is still running and is commissioned until October 2019. Practices can invite their diagnosed pre-diabetes patients to an annual check with a HCP. The consultation is flexible and what is discussed is at the clinician’s discretion. Obviously, diabetes prevention is the principal aim as current data indicates about 60% will develop Type 2 within the subsequent decade. This also represents an ideal opportunity to refer these patients into the National Diabetes Prevention Programme (NDPP) if they are interested.

The codes are:
Invitation - 9m90 (“impaired glucose tolerance invitation”) – payment £3
Attended - 6AC (“review of impaired glucose tolerance”) – payment £23

Please note just coding “pre-diabetes” in the consultation is not picked up for payment by the CCG CES searches.

NHS Diabetes Prevention Programme (NDPP) (“Healthier You”)

Berkshire is a 1st Wave site as you will know and the programme is still running, having commenced in June 2016. The local provider selected by NHS England is Reed Momenta (RM).

Reed Momenta have experienced some significant issues and the number of courses that started was initially small, particularly in Wokingham and Newbury areas. However, the situation has improved substantially in the last 3 - 4 months. We have also asked RM to improve feedback to practices, which has been poor. We await, with interest, data from the cohort of patients that complete the first nine month course.

NDPP is commissioned by PHE but GP Leads have been feeding back deficiencies in the process to both the commissioner and provider. RM have acknowledged that there have been significant problems but are working hard to rectify them. Jenny Willson took over as PHE lead from Edward Clintworth  last October and she has already visited many practices to meet with GPs, diabetes practice nurses and practice managers. Her email is jenny.willson@westberks.gov.uk (07766 924830).

Please continue to refer patients. The referral form is on DXS or the patient can self-refer by ringing 0800 092 1191, visit www.reedmomenta.co.uk/healthieryou,
Twitter @HealthierYouRM or Facebook/HealthierYouRM.
